Summary

Basic Function: The Registered Dietitian Nutritionist (RDN) provides comprehensive medical nutrition therapy including nutritional diagnostic, therapeutic, and counseling services as part of cardiovascular disease management for individuals and groups. Visits with patients address the following patient diagnosis and goals (but not limited to): dyslipidemias including complex lipid disorders, Phase II cardiac rehab, metabolic syndrome, diabetes, obesity, congestive heart failure, congenital heart disease, vascular surgery, hypertension, arrhythmia, peripheral artery disease and stroke. Patients range from late teen through adult.

RDNs coordinate with other care managers as needed to refer across the care management spectrum, and work with other team members such as physicians, nurses, social workers, and exercise physiologists to provide evidence-base, patient-centered care. Additional duties include preceptor for dietetic interns from the UM School of Public Health Dietetic Internship Program and research.

24 hours/week, weekdays only.

Hybrid position with some onsite time required at Domino's Farms Cardiology, Cardiovascular Center.

Organizational relationships

  • Reporting structure:
    • Ambulatory Care RDN Supervisor for professional oversight
    • Ambulatory Care Nutrition Manager
  • Liaison relationship with Domino's Farms Administrative Manager and Medical Director
  • Liaison relationship with interdisciplinary health care team for patient care populations of responsibility
  • Peer relationships with Registered Dietitian Nutritionists in work group.
  • Preceptor to Dietetic Interns and students, as able

Responsibilities*

PRIMARY DUTIES AND RESPONSIBILITIES
Patient Care

  • Conduct ongoing comprehensive nutrition assessments for patients with nutritional risks and/or for patients identified per ambulatory care clinic standards.
  • Incorporate evidence-based standards when interpreting patient related data.
  • Perform nutrition diagnosis after identifying the problem and clarifying the cause of the problem.
  • Develop and implement an individualized plan for nutrition intervention. Ground interventions through review of quality research and/or of medical nutrition therapy evidence-based practice, with consideration to addressing patient specific needs in the nutrition care plan.
  • Chart individualized nutrition treatment plan in the electronic medical record in a timely manner that is consistent with policy and procedures.
  • Recognize the varied needs of specific populations, including cultural, religious, and ethnic considerations.
  • Measure the effectiveness of nutrition interventions in reaching desired outcomes and implement changes as indicated.
  • Practice communication regularly with interdisciplinary ambulatory care medical teams.
  • Abide by Michigan Medicine policy for: the proper use of patients' protected health information, as well as the proper use of infection prevention precautions in patient care areas.

Business Functions

  • Provide nutrition service to the Adult Cardiology specialty clinics at Michigan Medicine for identified patient populations.
  • Implement charge capture by RDN for RDN services and facilitate scheduling patients as needed.
  • Provide quality service based on customer expectations and needs.

  • Maintain institutional and departmental productivity standards and practice effective time management.
  • Participate in developing nutrition-related policies, procedures, guidelines, protocols, and forms, particularly those associated with nutritional care in the adult ambulatory care setting.
  • Assist in achieving compliance with state and federal regulatory and accrediting agencies.
  • Participate in interdisciplinary meetings, task forces, and projects as appropriate.
  • Contribute to quality improvement efforts including collection of outcome data to help improve practice in the ambulatory care unit.
  • Participate in research studies and communicate findings through reports, abstracts, presentations, and publication as appropriate.

KNOWLEDGE AND SKILLS REQUIREMENTS

  • Able to work with a team to accomplish organizational goals.
  • Coordinate nutrition patient care services with the other Adult Specialty dietitians.
  • Communicate in a clear, concise, and diplomatic manner both verbally and in writing.
  • Implement critical thinking to solve problems.
  • Use technology such as word processing, spreadsheets, e-mail, and telecommuting platforms.
  • Demonstrate flexibility in job role to meet patient, department, and organizational needs.

Required Qualifications*

Minimum Education

  • Meeting the requirements and completing a didactic program in dietetics and a supervised practice program approved by the Commission on Accreditation for Dietetics Education (CADE) of the Academy of Nutrition and Dietetics.
  • Some experience in clinical dietetics.

Preferred Education

  • Master's Degree in Nutrition related field.
  • Two or more years of experience in cardiovascular medical nutrition therapy.
  • Experience in group facilitation.

Minimum Certification

  • Registered Dietitian Nutritionist with active registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics. Requires primary source verification.

Work Schedule

Work part-time, 24 hours/week; weekdays. 

Background Screening
