Summary

This is a full-time (40 hours/week) position in the Von Voigtlander Women?s Hospital supporting the Outpatient OB Service which includes coverage for the outpatient OB clinics, OB triage, Fetal Diagnostic Center, and OB PACU. This role specializes in psychosocial care needs of patients and families, including often providing urgent crisis intervention, fetal loss, coping with fetal anomalies, grief, trauma, safety, mood concerns during pregnancy and post partum, and other interventions.  This position will have a focus on the Fetal Diagnostic Center dealing with fetal anomalies, as well as being involved in clinic, triage and the PACU.

Responsibilities*

You will be responsible for:

  • Crisis intervention including but not limited to mental health crises, behavioral interventions (for patients or parents), and urgent needs assessments.
  • Assessment of coping, providing brief counseling/support, and connection to mental health resources.
  • Care coordination with medical team(s) re: adherence to treatment plan and/or quality of life.
  • Case consultation regarding treatment support needs for all multidisciplinary team members.
  • Assessment for barriers to care and connecting to appropriate community supports.
  • Education, assessment, and resource connection for patients receiving treatment.
  • Collaboration with community resources re: DHHS, CMH, CSHCS, etc.
  • Care coordination with CPS and foster care as well as consultation with CPT re: abuse/neglect concerns.
  • Education and support with adolescent to adult care transition, including guardianship and transition of care processes.
  • Psychosocial assessment re: complex care needs and management of treatment plans.
  • Providing information/education and referral re: insurance/CSHCS.
  • Partner with medical team to support patient in decision making or palliative care options.
  • Collaborate with the C&W Bereavement Program Manager.

Required Qualifications*

  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
  • A current license to practice social work in the State of Michigan.

Desired Qualifications*

  • Previous experience with families and children around issues of chronic illness, adjustment to illness and disability, learning/school issues, locating community resources.
  • Due to the nature of the variety of medical diagnoses associated with this population, proficiency in adjustment, loss, and bereavement issues is desirable.
  • Previous experience with medical and mental health social work.
  • Skilled in developing rapport and effectively communicating with a diverse patient population.
  • Efficient, accurate assessment of patient needs and effective care planning.
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.
  • Independent, sound decision-making is necessary for success in this position.
  • Effective collaboration with a large multi-disciplinary team.
  • Proficiency in keeping accurate and timely clinical records by computer.

Work Schedule

This is full time, 40 hour/week position. The schedule will be Monday-Friday 8am-4:30pm or 8:30am-5pm

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.
