Summary

This is a full-time (40 hours/week) position in the Division of Pediatric Genetics, Metabolism, and Genomic Medicine at Mott Children?s Hospital with hours of Monday-Friday 8-4:30pm. This social worker position will follow patients across the continuum of care providing social work support both during inpatient admissions and as part of the outpatient Genetics Clinic.

Responsibilities*

You will be responsible for:

  • Assess and support ongoing adjustment to diagnoses.
  • Provide support and resources to patients and families receiving new and life sustaining treatments for respective diseases.
  • Identify and assist with enrollment in specialized insurance programs for disabled children.
  • Care coordination with medical team(s) re: adherence to treatment plan, barriers to care and/or quality of life.
  • Case consultation regarding treatment support needs for all multidisciplinary team members.
  • Crisis intervention including but not limited to mental health crises, behavioral interventions (for patients or parents), and urgent needs assessments.
  • Assessment of coping, provide acute counseling/support, and connection to mental health resources.
  • Partner with medical team to support family in end-of-life decision making or palliative care options.
  • Assessment for barriers to care and connecting to appropriate community supports.
  • Case consultation regarding treatment support needs re: MSW, BSW, Child Life, RN, RD, Patient Financial Counselors 
  • Collaboration with community resources re: Make-A-Wish, Rainbow Connection, DHHS, CMH, CSHCS, etc.
  • Advocacy and extensive support with educational/school barriers re: 504 Plan or IEP and other related supports.
  • Care coordination with CPS and foster care as well as consultation with CPT re: abuse/neglect concerns.
  • Education and support with adolescent to adult care transition, including guardianship and transition of care processes.
  • Psychosocial assessment re: complex care needs and management of treatment plans.
  • Advocacy and coordination regarding obtaining or covering medications and DME.
  • Discharge planning and care coordination surrounding hospitalization and transitioning home.

Required Qualifications*

  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
  • A current license to practice social work in the State of Michigan.

Desired Qualifications*

  • Previous experience with families and children around issues of chronic illness, adjustment to illness and disability, learning/school issues, locating community resources.
  • Due to the nature of the variety of medical diagnoses associated with this population, proficiency in adjustment, loss, and bereavement issues is desirable.
  • Previous experience with medical and mental health social work.
  • Skilled in developing rapport and effectively communicating with a diverse patient population.
  • Efficient, accurate assessment of patient needs and effective care planning.
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.
  • Independent, sound decision-making is necessary for success in this position.
  • Effective collaboration with a large multi-disciplinary team.
  • Proficiency in keeping accurate and timely clinical records by computer.

Work Schedule

This is full time, 40 hour/week position. The schedule will be Monday-Friday 8am-4:30pm or 8:30am-5pm

Underfill Statement

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act.
